My girlfriend and I are splitting up and we have a 1 year old child. I am trying to download the forms online but I don't know which ones I will have to accomplish.

If you're not married, you need to establish paternity first. The parentage actions forms are the 200 series (FL-200, and FL-210). Along with your paternity petition papers, you can request custody orders with the 300 series forms (FL-301, 310, 311, etc.) Best is to hire a local family law lawyer to help you. If you can't afford a lawyer, you can go to the court's free self-help center, which you can find info about such services on the court's website.
