You can't find the forms because there is no such thing. What makes you think that you can unzip your pants, make babies and then refuse to pay support and expect the taxpayers to fund your sex life? That's ludicrous. Get counseling to find out why you have this lack of responsibility and please get a vasectomy. When you father a child, you have an 18 year responsibility. End of story. (One small exception is if someone else wants to adopt your child).

    The answer depends upon whether you are divorced, the subject for an action for paternity, or are otherwise involved in the childs life. If you are seeking to avoid child support obligations, the court will not allow you to give up your rights for that purpose.     Generally, the courts are not going to allow you to voluntarily terminate your parental rights. You might be allowed to do this if the child is being adopted by a step-parent, but otherwise the courts do not believe that it is in a child's (or society's) best interests for a parent to be allowed to self-terminate their parental rights and responsibilities.
