A 4 year old recently was reconnected with his father. There was a DNA test. Father now has visitation with the boy for the first time. The 4 year olds last name has now been changed to the father's without the mothers consent. What can she do to give him back his birth name?

The name generally cannot be changed without a court order. You would need to obtain a copy of the records which ordered the name change to determine if in fact the name has legally been changed and why the mother was not given notice of the name change. If you were not given notice of the court hearing, you may be able to set aside the order.
