How can I get my name on my child's birth certificate? 2 Answers as of July 20, 2011

My girlfriend is giving birth in LA (we are not married) and I cannot be present during labor. But we both want my name reflected as FATHERS name in the baby's birth certificate. I was told I need to be present to sign a consent form, but I am overseas. Is there a way to do this?

Ask the hospital if they can email the form to you and you return it by mail or email. Or fax to you and return. Each hospital makes their rules it seems.

You may sign a voluntary declaration of paternity prior to the time that your girlfriend gives birth to the child or immediately thereafter. Since you will not be present you would likely need to have the form notarized and sent to the county recorder.
